
MISSION STATEMENT
The mission of the Alliance Hospice Memorial Fund is to honor those who have died while on hospice. We can do this by providing those we continue to serve with a sense of their personal worth.
Some terminally ill patients and their families do not have the means to provide all that is involved in the dying process. Donations to the Alliance Hospice Memorial Foundation will help support extraordinary expenses that are out of the realm of health care but are critically important to the patient's and family's physical, emotional or spiritual comfort.

This assistance is available to patients and families served by Alliance Hospice and are evaluated based on need by the Alliance Hospice Foundation Board with input from the Interdisciplinary team serving the patient and family. This will be performed in a timely fashion to ensure the needs are met.
Giving Opportunities - Donating to Alliance Hospice Memorial Foundation
Alliance Hospice Memorial Foundation is a 501 c (3) nonprofit charitable organization as determined by the Internal Revenue Service. All contributions are tax deductible to the extent of the law.
Memorial Contributions - Send your contribution to your local Alliance Hospice office. Include the name of the person whose memory is being honored, the name and address of the person(s) who should receive an acknowledgement letter, your name, address and daytime phone number. Checks should be made payable to Alliance Hospice Memorial Foundation.
